YES I/we wish to enter the WINES OF GISBORNE REGIONAL WINE AWARDS as outlined in this document.

Entry Fee - $25 + GST per entry
Entry Quantity - Three (3) 750ml bottles of each wine as listed below
I/We understand the requirement that as selected winners of a category that I/we will supply at no cost to Gisborne Wine:

Twenty four (24) bottles of each wine for the Gisborne Regional Wine Awards Dinner on Friday May 25, 2012

WINE ENTRIES

I/we have posted a cheque to cover $25+GST($28.75) per entry
(to: Competition Director. PO Box 353, Gisborne, New Zealand).

Please send an invoice for payment by direct credit.

I/we have delivered wine to the GISBORNE WINE CENTRE
Shed 3, 50 The Esplanade, Inner Harbour, Gisborne.

All wine submissions must be received no later than Tuesday 15 May 2012
